Position Title: Health Educator

Grade Level: 15

FLSA: Exempt

Reports To:

Health Education Program Manager

Summary:

Provides healthcare education to support the efforts of all staff to include Wesley Nurse Health Ministries, as well as other agencies and organizations throughout the community to meet the Mission , Key Objectives and Core Values of the organization.

Essential Duties:

  • Coordinates health education and services with Physicians, staff, and healthcare providers.
  • Provides group or individual patient education and information to manage chronic health conditions.
  • Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient’s reaction to drugs, treatments, and significant incidents.
  • Initiates education and personal assessment according to the individualized needs of the patient, as prescribed by Physician and/ or hospital policy including patient and family instruction.
  • Prepares and disseminates educational and informational materials.
  • Participates in the development of programs designed to enhance health education.
  • Markets services of unit or clinic.
  • Participates in multidisciplinary team meetings.
  • Maintains educational resources.
  • Performs other duties as assigned.

Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education/Experience:

Graduate of an accredited Registered Nurse program. A minimum two years experience in patient education with adult level teaching experience preferred. Bachelor’s Degree (B.A.) in nursing preferred.

Language Ability:

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Fluent in English. Bilingual with Spanish preferred.

Reasoning Ability:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in mathematical or diagram form and deal with several abstract and concrete variables.

Certificates and Licenses:

Licensed Registered Nurse in the state of Texas .

Computer Skills:

To perform this job successfully, an individual should have working knowledge of the Internet, Outlook, PowerPoint, Electronic Health Records, and Word processing software and Excel spreadsheet software.

Other:

Ability to drive and have access to a car; maintain current driver’s license and auto liability insurance.

The work environment characteristics described here are representative of those a team member encounters while performing the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

While performing the duties of this job, the team member is exposed to medical office environment having direct contact with patients and clients. Position necessitates bending, stooping, twisting, turning, walking, climbing step stools, sitting and standing for periods of time. Must be able to lift and maneuver 25 pounds.
