Position Title: Social Worker I (LBSW)

Grade Level: 11

FLSA: Exempt

Reports To:

Social Work Supervisor

Summary:

Provides comprehensive case management supportive counseling, and referrals to individuals and families to support the efforts of all staff to include Methodist Healthcare Ministries, as well as other agencies and organizations throughout the community to meet the Mission , Key Objectives and Core Values of the organization.

Essential Duties:

  • Engages in basic generalist practice that includes interviewing, assessment, planning, intervention, evaluation, case management, information and referral, problem solving, consultation, supportive counseling, education, advocacy and community organization.
  • Maintains patient confidentiality and complies with all federal and state health information privacy laws.
  • Develops an action plan with goals, based on functioning level, physical/medical condition, support network, psychosocial and financial status based on long and short-term needs.
  • Conducts home visits, if needed, to complete Intake and Assessment
  • Develops and maintains effective working relationships with individual and/or family.
  • Makes referrals to community agencies and conducts follow up activities. Services as client liaison and advocate.
  • Refers clients who become ineligible for clinic medical services to alternative medical resources.
  • Assists patients/clients who are deemed ineligible for MHM services to access alternative resources.
  • Conducts individual and family training in psychosocial management skills.
  • Develops and maintains accurate case management and supportive counseling records.
  • Cooperates with and contributes to the development and implementation of a multidisciplinary treatment plan.
  • Maintains listings of community resources.
  • Cooperates with local medical, mental health, and community agencies.

· Performs other duties as assigned.

Qualifications:

Education/Experience:

Bachelor’s degree (B.A.) in Social Work and two years case management or related experience; or equivalent combination of education and experience.

Language Ability:

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Fluent in English. Bilingual with Spanish preferred.

Reasoning Ability:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Certificates and Licenses:

Licensed Baccalaureate Social Worker (LBSW) by the Texas State Board of Social Worker Examiners.

Computer Skills:

To perform this job successfully, an individual should have working knowledge of the Internet, Outlook, PowerPoint, Word processing software, Excel spreadsheet software and electronic health records.

Other:

Ability to drive and have access to a car; maintain current driver’s license and auto liability insurance.

While performing the duties of this job, the team member is exposed to an office and residential environment having direct contact with patients and clients. Position may necessitate bending, stooping, twisting, turning, walking, climbing step stools, sitting and standing for periods of time. Must be able to lift and maneuver 15 pounds.
